Carrier enrollment forms are found in the Forms section within each plan in EaseCentral. Forms are organized and available by plan type. EaseCentral's goal is to use the most up-to-date forms that apply for the majority of brokers and employers. Forms included, especially for ancillary plans may include all carrier's plan options, even if they don't apply to every group. Only the enrollments that are included in clients' portals will be mapped on the carrier forms. 


If a required carrier enrollment form is not available within the Forms section of a plan, new forms can be added and mapped within 4-6 weeks. Submit a ticket and attach a PDF version of the form to a HelpDesk ticket.  If forms are needed as a rush priority, please include time requirements in the ticket request and our team will prioritize requests accordingly.
