Many carriers require a completed waiver form if the employee waives coverage altogether or enrolls in another carrier's plan. EaseCentral will generate a waiver and enrollment form for the employee's medical election and declination. See below for an example of how this works.
If there are two carrier plan options:
- When an employee enrolls in Carrier #1's plan it produces both forms – enrollment for Carrier #1 and a waiver form for Carrier #2
- When an employee enrolls in Carrier #2's plan it produces both forms – enrollment for Carrier #2's plan and Waiver for Carrier #1's plan
- When an employee waives both Carrier #1's and Carrier #2's plans a waiver form is produced for both plans
When creating medical plans be sure that an enrollment form is included in each plan. EaseCentral will populate the enrollment or waiver portion of the form based on the employee's choice. After the employee has made their elections and completed their enrollment process they are reviewing and signing their forms. Depending on how they enrolled or waived, they will view an enrollment form populated with the plan information they elected. They will also view the declination or waiver portion completed for any plan(s) they did not elect. The benefit of this feature is that carriers and employers will have record of all medical plans offered to an employee along with an enrollment and/or declination for all plans.
This feature is already enabled in EaseCentral. On the Customize > General tab, the Yes, Generate Multi-Carrier Waiver Forms option will be defaulted so waiver and/or enrollment forms will be completed for each carrier.
Should this feature need to be disabled simply un-check the Yes, Generate Multi-Carrier Waiver Forms option.
