Enrollments into plans that have specific dollar coverage amounts can be populated in EaseCentral via import. When importing these enrollments be sure that the amount of coverage is listed in the "Import ID <6 digit code>" column on the import spreadsheet. Do not list “Enrolled.” for salary/volume based benefits. Please review the plan types and examples below for importing guidelines:

 

Life/AD&D, Life, AD&D plan types:

Flat amount or Increments of coverage

Example benefit amount(s) based on plan rules: $10,000; $25,000; $100,000

Value to include on spreadsheet: $10,000; $25,000 or $100,000

 

Increment of salary (X Earnings)

Example benefit amount based on plan rules: 2x salary

Example employee salary: $35,000

Value to include on spreadsheet: $70,000


Long Term Disability plan types:

Increment of salary (X Earnings)

Example benefit amount based on plan rules: 60% of salary to $10,000 max

Example employee salary: $35,000

Value to include on spreadsheet (35000/12*.6): $1,750 


Short Term Disability plan types:

Increment of salary (X Earnings)

Example benefit amount based on plan rules: 60% of salary to $500 max

Example employee salary: $35,000

Value to include on spreadsheet (35000/52*.6): $403 


IMPORTANT: If the employee's coverage amount goes over the plan maximum, please enter the plan maximum as their coverage amount. For example, if an employee's salary was $250,000 and they were enrolling in the above plans, please load their amount of coverage as $10,000 for LTD and $500 for STD. Do not load $12,500 for LTD nor $2884 for STD as those amounts are over the amount allowed per the plan.
